I understand that participating in the AbbVie World of Well-Being program is strictly voluntary, is not a requirement or expectation of my job and is conducted solely for the benefit of my own health, morale and welfare. I understand that AbbVie derives no benefit from my participation in the program.

The personal information I provide will be used to administer the program and may be linked with other demographic information about me that is maintained by AbbVie systems to further conduct analytics and provide me with incentives for which I am qualified. The personal information I provide will also be used by AbbVie’s contracted third-parties to send me information and surveys by email about AbbVie World of Well-Being. I understand that these providers will also use this information to support the program and to conduct analytics on participation. I understand that identifying information about me will be made available on the WOW News Feed with my submission, including all stories, comments, interviews/surveys, photographs, video and/or audio (referred to collectively as the “Submissions”) posting on the public AbbVie World of Well-Being website. I understand and authorize my information to be transferred to countries outside of my own to facilitate the above stated purposes. Requests to be removed from the mailing list or for a copy of this information can be submitted to abbviewow@abbvie.com.

AbbVie assumes no responsibility, in whole or in part, for any injury, loss, or damage incurred as a result of participation in the AbbVie World of Well-Being program. I assume full responsibility for all damages, injuries, and/or losses of personal property while participating. I understand that the AbbVie World of Well-Being program will not determine whether an exercise program or dietary change is medically appropriate. It is my responsibility to consult with my personal physician regarding these matters, and I am aware that I should consult a physician before participating in any type of physical activity. I certify, to my best knowledge, that I am not aware of any reason, medical or otherwise, that would prevent me from participating in the AbbVie World of Well-Being program.
